Arajet transports Service, Assistance, Rescue or Search Dog to and from the United States.
We recommend checking with the appropriate health authorities before your trip since pet must have all required health documentation as specified by both the country of origin and the destination.
General Requirements:
• DOT Service Animal Air Transportation Form: Required for all service animals traveling by air.
• CDC Dog Importation Form: Required for entry into the U.S.
• Certificate issued by a licensed veterinarian stating that the dog is free from Screwworm, dated within 5 days prior to travel.
Vaccinated in the United States:
• The completed DOT Service Animal Air Transportation Form.
• The CDC Dog Importation Form.
• One of the following documents:
- A USDA Export Health Certificate, issued by the U.S. Department of Agriculture.
- A valid Rabies Vaccination Certificate issued in the U.S.
Was Vaccinated outside the United States:
• The completed DOT Service Animal Air Transportation Form.
• The CDC Dog Importation Form.
• A foreign Rabies Vaccination Certificate and microchip information.
- The foreign certificate must comply with CDC standards.
According to CDC regulations if your dog was vaccinated against rabies outside the U.S. they may only enter the country through airports with a CDC-registered animal care facility, out of the list Arajet currently operates in Miami (MIA).
